PLEASE NOTE: Everyone needs to fill out our Customer Information /Boarding Agreement Form (see below) and EMAIL it back to us along with your pet(s) proof of vaccinations, before a reservation will be held. DO NOT bring in the paperwork, it will NOT be accepted!! ALL paperwork must be emailed to us. (opboardingkennel@sbcglobal.net) Please Note: You will be scheduling a specific drop off and pickup time on our form. Please be punctual as these are appointments. If you show up too early, we may ask you to come back at you scheduled time.
